End-of-Life Pathway
Additional support for people who have been assessed as needing end-of-life care and who wish to remain at home, with comfort and dignity, during the final stage of life.
What it funds
The pathway funds the same kinds of services available under Support at Home, but delivered at a higher level where that's needed to meet increased care needs during this time.
How much funding is available
Approximately $25,000 is available over a 12-week period, and some people may receive support for up to 16 weeks.

Can it be used alongside my ongoing budget?
No — and this is an important difference from the Restorative Care pathway. While you're on the End-of-Life pathway, this funding replaces your ongoing Support at Home budget. Your services are funded from the End-of-Life budget for the duration of the pathway.
How it's funded
The End-of-Life budget is used first; grandfathered HCP unspent funds are used next if it's exhausted. Care management is billed directly against the End-of-Life budget, with no separate account. Try the funding tool →
