Care Management funding
Care management is the behind-the-scenes work that makes sure your services keep meeting your goals and changing needs — planning, coordinating and reviewing your support.
What it includes
Care management covers the coordination activities that keep your care working well, such as:
- Care planning and goal reviews
- Coordinating your services and communicating with providers
- Monitoring your budget
- Reviewing your support plan as your needs change
Why it matters
Care management helps ensure your services stay appropriate, your funding is used effectively, your goals stay current, and any change in your needs is picked up early — before it becomes a problem.
How it's funded
For ongoing services, around 10% of your quarterly budget is allocated to care management automatically — you don't need to budget for it separately.
When you're on the Restorative Care or End-of-Life pathways, care management is billed directly against that pathway's budget instead. There's no separate care management account in those cases.
Good to know
This is why your "available for services" figure may look a little lower than your full quarterly budget — the care management portion has already been set aside.
