Top five mistakes to avoid when transitioning to Support at Home

From 1 November 2025, the Support at Home program will replace the current Home Care Packages (HCP) program. Whether you’re a Grandfathered or Hybrid client, early preparation helps you avoid stress and unnecessary costs.

1. Misunderstanding client contribution rules

One of the most common Support at Home mistakes is misunderstanding client contributions. Under the new rules:

  • Hybrid clients (entered HCP after 12 Sep 2024) must contribute to most non-clinical services.
  • Grandfathered clients may be exempt under the “no worse off” rule.

2. Not reviewing your Trilogy Care care plan

Support at Home introduces a defined Service List with strict inclusion and exclusion rules. Services not on this list won’t be subsidised.

3. Ignoring unspent funds

Unspent funds in your new quarterly budget can roll over, but only up to $1,000 or 10%, whichever is higher. Excess funds are forfeited.

Many people will move to a tiered client contribution model for services like domestic assistance and personal care. Clinical services remain fully subsidised.
