What does a care partner actually do under Support at Home?

Under the new Support at Home program, you will still have someone in your corner to help make sense of your funding and turn it into real support at home. At Trilogy Care, that person is your Care Partner.
Your Care Partner is your main point of contact for your Support at Home plan. They help you understand your budget, decide how to use it, and keep your services aligned with your goals and routines. Their focus is to make sure your support feels practical and helpful in your everyday life, not just good on paper.
A Care Partner also gets to know what matters most to you. They listen to how you like to live, what you want to keep doing for yourself and where you would like more help. This helps them suggest supports that fit your lifestyle, not just your care needs.
What your Care Partner helps with
A Care Partner can help you to:
Understand how your Support at Home funding works
Plan the right mix of services for each week
Check that your supports still match your needs
Sort out questions or hiccups with services or invoices
They are there when things change. If your health shifts, your routine changes, or you feel you need more or less support, your Care Partner can review your plan with you and adjust your services so they still fit.
If you want more hands-on help, your Care Partner can also work alongside a care coordinator who does more of the organising and follow up on your behalf. This can be useful if you would rather have someone else handle the day to day arranging of services.
How your Care Partner works with your family
Many people want family or friends involved in their support. Your Care Partner can include your partner, children or other important people in your planning conversations, if you are happy for them to be there.
They can help everyone get on the same page about what support is available, who is doing what and how the funding is being used. This can reduce confusion and make it easier for families to share the caring load.
If you have someone who acts as your authorised representative, your Care Partner can also work with them to make sure your choices and preferences are at the centre of every decision.
How are they different from a traditional case manager?
A traditional case manager often decides which providers you use and arranges everything for you, with less focus on your own choice and control.
Your Trilogy Care Partner is there to guide and support you, while still giving you room to choose who comes into your home, when they visit and what they do with you each visit. They focus on making your funding practical, flexible and centred on what matters most to you.
Over time, your Care Partner checks in and helps you adjust your support as your needs, goals or circumstances change, so your plan does not stand still while your life moves on.
