Welcome to Support at Home with Trilogy Care

From 1 November 2025, a new chapter in aged care begins. The Support at Home program will replace the current Home Care Packages program. With Trilogy Care as your Care Partner, your support will continue under the new aged care program with stability, clarity and care.

These changes are part of national aged care reform designed to deliver greater transparency, consistency, and sustainability. Most importantly, the reforms aim to support you to live safely and independently in your own home, with services that reflect your needs and preferences.

Whether you’re continuing with us or joining Trilogy Care for the first time, we’re here to support you.

What’s changing and what’s staying the same

If you’re already receiving services, your support will continue without interruption under the new program. You’ll still work with the same provider, support team and a care plan that reflects your goals. While some details around budgets and contributions are changing, key principles of choice, dignity and quality care will remain the same.

If you’re a Grandfathered client, you’re protected under the “no worse off” principle. This ensures your contribution rates will not increase under Support at Home.

If you’re a Hybrid client, new contribution rates will apply from 1 July 2025. Trilogy Care will provide guidance to help you understand what this means for your budget and service access.

Key things to know about Support at Home

  • Support at Home launches on 1 November 2025 and replaces the current Home Care Packages program as the new aged care program to be used across Australia

  • Grandfathered clients will automatically transition, with no changes to their provider or current funding level

  • Your annual funding will be distributed in quarterly budgets, making it easier to plan ahead

  • A 10% care management fee applies across the board — no more, no less

  • Package management fees are ending; administration costs will be included in service pricing

  • Services will now fall under a new three-category structure: Clinical care, Independence support, and Everyday living support

  • Unspent funds can still roll over each quarter, with new limits to help ensure ongoing access to care

  • A separate Assistive Technology and Home Modifications (AT-HM) scheme will provide funding for equipment and home changes — unspent funds must be used first

  • Restorative Care and End-of-Life Pathways offer targeted, short-term support if your needs change

  • f you're a Hybrid client, new contribution rates will apply from 1 July 2025. We'll make sure you're fully informed and supported
