The Assistive Technology and Home Modifications (AT-HM) scheme is part of Support at Home and is expected to begin in November 2025. It creates a dedicated pool of funding for equipment and household mofdifications so clients don’t need to reduce care hours to pay for things like mobility scooters, personal alarms, grab rails or bathroom adjustments. Here’s how to prepare and access the support when it opens.
What AT-HM will cover?
AT-HM is designed to fund a wide range of supports, including:
- Assistive technology such as walking aids, shower chairs, personal alarms and mobility scooters
- Minor and major home modifications like ramps, grab rails, bathroom adjustments and stair access solutions
- Clinical input and training required to prescribe or safely use equipment
Step-by-step: how to access AT-HM funding
- Confirm your goals and risks
Think about what you want to improve at home, such as safer showering, easier entry and exit, or getting out into the community - Request a support plan review
Contact My Aged Care or speak with your care partner to update your support plan so it clearly lists your goals, safety risks and the equipment or modifications you may need - Arrange the right assessment
Some items require an allied health prescription or assessment. Your provider can help organise an occupational therapist, physiotherapist or other clinician so requests meet program requirements - Gather quotes and product details
For bigger purchases and modifications, you will usually need quotes and product specifications. Choose reputable suppliers and make sure items meet your goals and fit your home - Submit your request through your provider
Your care partner will help lodge the request, track approvals and coordinate delivery, installation and training so equipment is set up safely - Keep records
Hold onto assessments, quotes and invoices so you can demonstrate value for money and maintain your equipment over time
Funding limits to be aware of
A lifetime funding cap of $15,000 per person has been proposed for AT-HM ($15,000 for Assistive Technology and a separate $15,000 for Home Modifications). If you need more than one major item or a mix of equipment and building work, plan your purchases carefully. For example, a mobility scooter can cost several thousand dollars and a basic bathroom upgrade can exceed ten thousand dollars. Prioritise items that reduce risk and deliver the biggest day-to-day benefit, then sequence anything else over time.
What you can do before November 2025
ou don’t need to wait for launch to get started:
- Complete a simple home safety check to identify slippery areas, steep steps, poor lighting or hard-to-reach storage
- List equipment or changes that would make daily life easier, such as grab rails, a handheld shower, threshold ramps or improved lighting
- Talk to your provider about assessments you might need so you are ready to move quickly when the scheme opens
- Use any unspent Home Care Package funds strategically where appropriate, and keep documentation tidy
- Collect brochures or quotes for items you are considering so comparisons are straightforward later
