Support at Home is the new government-funded aged care program that will replace Home Care Packages from 1 November 2025. It’s designed to give older Australians more control, more flexibility, and better value when accessing care to stay independent at home.
If you’re not already receiving aged care services and think you may need support soon, applying for Support at Home is the first step. This guide explains who can apply, how the process works, and how Trilogy Care helps you make the most of your funding.
What is Support at Home?
Support at Home introduces a simpler and more flexible way to access care. Instead of daily subsidies, you will receive a quarterly budget based on your care needs. That budget can be used for services like personal care, domestic help, meals, transport, nursing, and more.
Care is grouped into three service categories: Clinical, Independence, and Everyday living. You will work with a provider to organise services, or you can self-manage with support from Trilogy Care to get more hours from your available budget.
Who is eligible?
You can apply if you:
- Are aged 65 or older, or 50 or older for Aboriginal or Torres Strait Islander people
- Need help with daily tasks to live safely at home
How to apply
Register with My Aged Care
Call 1800 200 422 or visit myagedcare.gov.au to start the process. You will need your Medicare card and some basic information about your current health and daily routines.
Complete an aged care assessment
You will be contacted by the Single Assessment System to organise a home visit. During this visit, an assessor will talk with you about your needs and goals and determine your eligibility for Support at Home funding.
Receive your funding decision
If approved, you will receive a Notice of Decision confirming your classification and quarterly budget. This means you can now choose a registered provider and begin arranging services.
