When to ask for a reassessment

Step-by-step to request a higher classification

  1. Talk to your care partner and review your Support Plan
    Book a check-in with your Trilogy Care care partner. Together, update your Support Plan to reflect what has changed and which services you now need across clinical, independence and everyday living

  2. Gather the right evidence
    Provide recent documentation that shows increased need, such as:

    • GP or specialist letters, discharge summaries and medication changes

    • Allied health reports (physio, OT, speech pathology) with specific recommendations

    • Falls logs, wound charts, continence plans or behaviour observations

    • A month of rosters or invoices showing your current budget is not sufficient

  3. Request a Support at Home reassessment
    Your care partner can submit a request through the Single Assessment System. If approved, you’ll receive a higher Support at Home classification and an adjusted quarterly budget from the decision date

  4. Ask about short-term pathways if you need extra help now
    Goal-focused, short-term supports may sit alongside your budget when you meet the criteria. Your care partner will advise what is appropriate and how to apply

  5. Consider Assistive Technology and Home Modifications
    If equipment or home changes would keep you safe and independent, ask your care partner to check eligibility for funding options and rules

Plan services across the full quarter and keep good records. Statements, invoices and waitlist notes show real-world demand, which strengthens your case for a higher classification. Your care partner will also explain how contribution settings apply to your services so there are no surprises.
