The Support at Home program is a key reform in Australia’s new aged care program, designed to deliver greater consistency, transparency and choice for older Australians receiving care at home. It replaces the Home Care Package program from 1 November 2025.
This transformation is more than just a name change—it’s a shift in how aged care services are funded, structured and delivered.

What is Support at Home?
Support at Home is a new aged care program starting on 1 November 2025. It will replace the current Home Care Package (HCP) program and is designed to improve in-home care by offering more flexibility, transparency and support for older Australians to live independently at home for longer.
Will I automatically move to Support at Home?
Yes. If you’re currently receiving a Home Care Package, you’ll automatically transition to the Support at Home program at the same funding level. If you’re on the National Priority System (waiting list), your funding will commence at the level you were approved for when it becomes available.
What are the key benefits of the Support at Home program?
The Support at Home program introduces several improvements over the current Home Care Package system:
- More funding – The maximum annual funding will increase from $61,440 to $78,000
- Care partners – A registered provider will help manage your care and services
- Better pricing – No separate administration or management fees
- Faster access – Wait times for funding will reduce to three months by 2027
- Assistive Technology & Home Modifications – Up to $15,000 upfront for things like walking aids and bathroom modifications
- Short-term support – Additional restorative care funding and a new End-of-Life Pathway to help people remain at home in their final months
How will my funding work?
- Your funding will be divided into quarterly budgets (every 3 months)
- If you don’t spend all your funds in one quarter, you can carry over up to $1,000 (or 10%) to the next quarter
- Funds will be managed by Services Australia to ensure transparency
What services are covered under Support at Home?
There are three service categories under the new home care program:
- Clinical support (e.g., nursing, physiotherapy) – Fully funded by the government
- Independence support (e.g., personal care, assistive technology) – Moderate contributions required
- Everyday living support (e.g., cleaning, gardening) – Higher participant contributions apply
Will I have to pay for services?
- The government will cover 89% of aged care costs
- You may be asked to contribute to non-clinical services (like personal care or domestic assistance) based on your income and pension status
- People on low incomes will continue to receive financial protections
What happens if I need assistive technology or home modifications?
The Support at Home program offers upfront funding of up to $15,000 for eligible participants. This covers:
- Assistive technology (e.g., wheelchairs, walking frames)
- Home modifications (e.g., grab rails, ramps)
A health professional may need to prescribe certain modifications
Will I still have care management support?
Yes. 10% of your quarterly budget will be set aside for care management, ensuring you receive help to coordinate and adjust your services as needed.
What if I need short-term or end-of-life care?
Support at Home includes:
- Restorative care pathway – Up to 12 weeks of support to help regain independence
- End-of-Life pathway – Up to $25,000 over 3 months for those with less than 3 months to live, to support a dignified end-of-life at home
How will payments to providers work?
- Your funding will be managed by Services Australia
- You only pay for services you receive (no upfront fees)
- Your provider will collect your participant contributions directly from you
Do I need to be reassessed under the Support at Home program?
No, not unless your care needs change.
If you are already approved for or receiving a Home Care Package, you will move automatically to Support at Home. A reassessment may only be required if your care level needs to be reviewed under the new classification system.
What are the new funding levels?
Support at Home introduces 8 funding levels, replacing the 4 current ones under the Home Care Package program. There will also be two short-term pathways:
- Restorative care pathway – For temporary intensive care to improve independence
- End-of-Life pathway – For those with less than 3 months to live
Funding levels will be based on your care needs and assessment outcomes, with a priority rating of high, medium or standard.
What is a Grandfathered Client (under Support at Home)?
A Grandfathered client is someone who, as of 12 September 2024, was:
- Receiving a Home Care Package, or
- On the National Priority System (NPS) waiting list, or
- Assessed as eligible for a Home Care Package
These clients are protected by a “no worse off” principle under the Support at Home reform.
What this means for you:
- You’ll pay the same or less than you would have under the current Home Care Package system
- If you’re a full-rate pensioner who pays no fees now, you will not pay fees under Support at Home
- If you are currently contributing fees, the rate may change—but you will not pay more than you currently do
- If you are later reassessed (after 1 July 2025), your contribution may be adjusted based on your services, but only if it results in a fair outcome
What is a Hybrid Client (under Support at Home)?
A Hybrid client refers to someone who was approved for a Home Care Package between 13 September 2024 and 30 June 2025 (inclusive).
What this means for you:
- You will move to the standard Support at Home arrangements on 1 November 2025
- Your care and contribution arrangements will follow the new Support at Home pricing and fees framework from the date of transition
What is a Transitioned Client?
The term Transitioned client is used to collectively refer to all existing Home Care Package recipients who will move from the Home Care Package program to the Support at Home program on 1 November 2025. This includes both Grandfathered clients and Hybrid clients.
What this means for you:
- If you are currently receiving a Home Care Package, you are considered a Transitioned client when you move to Support at Home on 1 November 2025
- Your transition will be automatic, with no need for reassessment
- Your care level and services will continue, but your fee and contribution arrangements will align with your specific category (Grandfathered or Hybrid)

When does Support at Home start?
The Support at Home program will now start on 1 November 2025. This new date follows a brief deferral from the original 1 July 2025 start. The current Home Care Packages (HCP) program will continue until 31 October 2025, ensuring there’s no disruption to your care.
Why was Support at Home delayed?
The program was delayed until November in order to give older people, their families, and providers more time to prepare. The government listened to sector feedback and made the decision to ensure the transition is safe, well-managed, and supports everyone involved. It’s a positive move that puts client care first.
Is the delay good for aged care clients?
Yes, the short delay to the aged care reform is good for clients. It gives you and your family more time to understand the upcoming Support at Home changes and feel confident about what’s coming. It also allows providers like Trilogy Care – who are already well prepared – to continue refining support systems and resources. This means you’ll be even better supported when the program begins on 1 November 2025.
What does the extra time allow Trilogy Care to do?
The extra time allows Trilogy Care to fine-tune our systems, train our team, and work closely with you to prepare for Support at Home November. We’re reviewing every client’s care plan to make sure it transitions effectively under the new model, and we’ll continue to share updates and support you every step of the way.
Will there be more changes to the aged care legislation?
Yes, the government is continuing to finalise key details of the new Aged Care Rules, which support the operation of the new Act. These updates are expected in the coming months, and we’ll keep you informed about any changes that might affect your care or services. Trilogy Care is across all Support at Home November developments, so you don’t need to worry.
Will Home Care Packages continue during the delay?
Yes, Home Care Packages (HCPs) will continue until 31 October 2025. You’ll keep receiving your care, funding, and support as usual. There are no changes to your services or fees during this period, and you don’t need to do anything differently.
Will my fees change because of the delay?
No, your current fee arrangements will stay the same until 31 October 2025. If you are a grandfathered client, the ‘no worse off’ principle will apply beyond 1 November 2025.
If you are a hybrid client, you will transition to the new contribution framework on 1 November 2025. Trilogy Care will explain how this affects you before the transition.
Will my care or services change before Support at Home begins?
No, there will be no changes to your current services before 1 November 2025. You’ll continue receiving the same support through your Home Care Package, with the same funding and arrangements in place. If your care needs change, your care partner will review your care plan and discuss any necessary updates with you.
Do I need to do anything right now?
No, you don’t need to take any action at this stage. Trilogy Care will handle all the changes behind the scenes and will keep you informed. We’ll reach out to discuss any further Support at Home updates that affect your funding or services well before anything changes.
Will I lose my unspent Home Care Package funds?
No, your unspent funds will not be lost. You will be able to carry over unspent Home Care Package funds into the Support at Home program. However, rollover will be subject to new rules and limits under the Support at Home model. We will explain these conditions to you once the government releases the final rules.
What if I’m waiting for a higher-level package?
If you’re already on the national waitlist for a higher-level package, that won’t change. You’ll continue moving through the queue under the current system. If your new level is approved after 1 November 2025, you’ll move to that new funding level under Support at Home, and your protections (like grandfathering) will still apply if you’re eligible.
