From 1 July 2025, the current Home Care Packages program will be replaced by the Support at Home Program. As a Self Managed Plus Grandfathered client, you’re protected under the ‘no worse off’ principle- but that doesn’t mean everything stays the same. 

Join Trilogy Care for a practical overview of what’s changing, how it affects you, and what you can do now to prepare.  

We’ll explain what the transition means if you’re self-managing, what you’ll keep, what may change, and how we’ll support you to continue receiving the care you choose, at home.  

This webinar is ideal for anyone currently self-managing a Home Care Package on a Trilogy Care Self Managed Plus account.
