Who the pathway is for
The pathway is for people who prefer to stay at home in their final stage of life. A medical practitioner or nurse practitioner confirms eligibility using the program form, and the plan focuses on the person’s goals and what matters day to day.
How access works
Access can be prioritised. Once the clinician confirms that the pathway is appropriate and completes the form, an aged care assessor can review the plan so in-home services can start as soon as possible. The aim is to reduce administrative steps and put the right supports in place promptly.
What support can look like
Every plan is different. In practice, supports may include personal care, nursing, domestic assistance, basic equipment or equipment hire, and short-notice respite. Services are tailored to the client’s needs and scheduled around family routines. Plans can be adjusted as circumstances change.
