Trilogy Club Care is an innovative program that partners with your club to assist club members in navigating the Home Care Package program. Trilogy Club Care will help you, the club member, to manage your Home Care Package so that you can receive the support and services you need.

What is a Home Care Package?

The Home Care Package program was established by the Australian Government to assist older Australians to live independently in their own home.

The program is designed to provide flexible, tailored support according to a person’s health care needs. Funding is allocated after formal assessment and can include services to help a person stay.

About Trilogy Care

We design the care plan and budget with the client and then help them to go directly to personal carers, clinical and support workers. By doing so, clients can reduce the rates for care and services and maximize their care hours.

Trilogy Care handles the cumbersome compliance, financial reconciliations, and government reporting so that you have the peace of mind. Our model of support has been a disruptor to the Aged Care sector by providing clients with a lower cost and more efficient option for managing the Home Care Package than the traditional providers.
